Applying.
Anyone who has been through the experience of obtaining a residential care placement will tell you that the paperwork can be daunting.
Each residential care facility prefers to use its own application form.
Many other complex forms may need to be filled in, including the Financial Assessment form, Power of Attorney or Appointment of a Nominee form.
Knowing what to do, and how to do it correctly, will help you avoid long delays in getting your assessment processed.
Seniors Pathway can help you by:
- Accurately completing all forms and assessments and lodging them with each selected residential care facility and other relevant organisations.
- Contacting the selected residential care facility to confirm receipt and assist with early acceptance.
